One output of the ITD Target State Design is a high level plan which must be converted into a formal programme of work with all the typical transformation domain disciplines (metamorphic changes) , combined with improvement measures (output of an ITD improvement assessment and tactical implementation plan). This is likely to commence with an extended and more thorough business case that sets out the costs, risks and benefits in more detail along with the critical success factors for the transformation and considering the overall changeability of the organization.

The execution plan is created consisting of all activities with their accountable and responsible owners. Dependences on resources and other activities are captured and addressed. The journey is planned in stages of no more than a few months, each stage delivering tangible and measureable benefits in a well balanced mix within the plan, design, build and run domain. Priority is according business criticality, return on investment and immediate evidence of improvement, with maturity (as defined in the maturity model) improving at each pass to the level desired for a specific change.

Socialisation and approvals must be gained from all major stakeholders. This is continuous like your transformation management of change (MoC), not only including persuasive staff information and communication actions to prepare your organization on the right time and the right content, but also intensively planning and implement the ‘inner’ shifts of mind-set and culture. The ‘external’ implementation of new structures, processes, roles or deliveries will not produce the intended benefit if it is not acknowledged by the staff.

Possible Deliverables:

Transition Execution Plan and Project Inter-dependency Map
Which describes concrete activities with project briefs of what to implement, scope, deliverables, timeline, and budget. The transformation book defines how the execution will be fulfilled according which principles, approaches, which constraints and ruleset is to be respected. The interdependency chart visualizes the relations of the projects over the execution phases .

Transformation and Transition Portfolio
Which represent the Program management  instrument to manage, control and report the progress of each individual project and their status, consolidated to the overall transition scope and its progress. It is the source for all reports and presentations to the leadership teams.

Operational Communication Plan
which describes the concrete communication activities to manage the change (MoC). The descriptions include the scope of the messages, objectives of the information, target groups, time line.  Further the methodologies of how the information will be transported and the trainings are implemented, like gallery walks, coffee corners, news paper, mandatory web trainings, etc.
